HEALTH SCREENING FORM SUBMISSION​

All employees are encouraged to get an annual preventative physical with their primary care physician which includes a biometric health screening to get important updates on your health status. This requirement can be completed by self-submitting your results via your private platform account or faxing a Health Screening Form to The McCahill Group via the instructions at the top of the form. Below are the steps to log your results in your private platform account. None of your personal health information is shared with UASI.

DEADLINE: OCTOBER 31, 2023 @ 5PM EST

SELF-SUBMIT PROCESS:

1. Log into your private platform account.
2. Click "Health" tile at the top of the page.
   Note: for privacy and security of your personal health information you may be asked to log in a second time.
3. Log the results of the categories provided and upload proof of your physical.
   Proof may include a copy of your Health Screening Form or screenshot of your visit details from your health system's portal (ex: MyChart).​​

FAX PROCESS:

• Print or send copy of Health Screening Form to your primary care physician prior to your annual physical visit.
  • Top portion of form must be filled out by employee.
• Request that your doctor's office submit the form via the instructions at the top of the form.
